Once you've downloaded, printed and filled out the application please fax it to us at: 802-445-7571.

PC: Place your cursor on the word "click here" or the PDF Icon and then Shift-Left click (Netscape) or right click on the mouse (Microsoft Internet Explorer). Select "Save Target As" from the menu. You will be asked where to save the file.

MAC: Place your cursor on the word "click here" or the PDF Icon and then press and hold the mouse button down until a menu pops up, select "Download Link to Disk" (Explorer) or "Save This Link As" (Netscape). You will be asked where to save the file.

Once you have downloaded and installed the free Adobe Acrobat Reader, you may open, view and print the files in PDF format.
